What Is the Thyroid Gland?

The thyroid gland is a butterfly-shaped organ located at the base of your neck. It produces hormones that help regulate your metabolism, heart rate, and energy levels. Because of its location near the voice box, windpipe, and swallowing muscles, thyroid problems can sometimes cause symptoms that fall under ENT care.

What Causes Thyroid Problems?

Hormonal imbalances may result from autoimmune conditions such as Hashimoto’s thyroiditis (underactive thyroid) or Graves’ disease (overactive thyroid). Structural issues like nodules, goitres, or cancer may develop due to genetic factors, iodine deficiency, inflammation, or unknown causes. Some thyroid conditions are discovered during routine health checks, even when no symptoms are present.

Common Thyroid Conditions We Treat

Thyroid conditions can vary widely—from harmless nodules to more serious concerns like cancer or hormonal imbalances. Below are some of the more common thyroid conditions we see and manage in our clinic.

Thyroid Nodules

These are lumps that form within the thyroid gland. Most are non-cancerous and don’t cause symptoms, but some may grow large enough to be felt or seen, or may need a biopsy to rule out cancer.

Thyroid Cancer

Thyroid cancer can present as a painless lump in the neck. Early diagnosis and surgery offer a good chance of cure, especially for common types like papillary and follicular thyroid cancer.

Goitre (Enlarged Thyroid Gland)

A goitre is an overall swelling of the thyroid gland. It may cause visible neck swelling or pressure symptoms such as difficulty swallowing or breathing.

Hyperthyroidism (Overactive Thyroid)

This occurs when the thyroid produces too much hormone. Symptoms may include weight loss, palpitations, tremors, and heat intolerance. 

Hypothyroidism (Underactive Thyroid)

This is when the thyroid doesn’t produce enough hormone, leading to fatigue, cold intolerance, or weight gain. 

 

Symptoms of Thyroid Problems

Thyroid issues can cause a range of symptoms, depending on whether the problem is structural, hormonal, or both. Some signs to watch for include:

  • A lump or swelling in the front of the neck
  • Voice changes or hoarseness
  • Difficulty swallowing or a sensation of pressure in the throat
  • Persistent cough not linked to infection
  • Enlarged lymph nodes in the neck
  • Fatigue, weight changes, or heat/cold sensitivity

Diagnosis: How We Evaluate Thyroid Conditions

At The ENT Specialist Centre, we use several tools to assess thyroid problems and determine whether further investigation or treatment is needed:

Neck Examination

A physical examination helps detect any swelling, asymmetry, or lumps in the thyroid area. The size, texture, and mobility of a nodule can provide useful clues about its nature.

Ultrasound Scan

This imaging test allows us to visualise the thyroid gland in detail, assessing the size, shape, and internal structure of any nodules. It also helps identify features that may suggest whether a nodule is benign or suspicious.

Fine Needle Aspiration Cytology (FNAC)

FNAB involves using a thin needle to collect cells from a thyroid nodule, which are then examined under a microscope. It is a key test to determine whether a nodule may be cancerous.

Thyroid Function Tests

These are blood tests that measure hormone levels produced by the thyroid gland, including TSH, T3, and T4. They help guide the overall diagnosis and management plan.

Treatment Options for Thyroid Problems

Treatment for thyroid conditions depends on the underlying issue—whether it’s a hormonal imbalance, a structural concern, or a potential malignancy. Here’s how we typically approach each condition:

Thyroid Nodules and Goitre

Thyroid Cancer

  • Surgery to remove part or all of the thyroid (thyroidectomy)
  • Referral for radioactive iodine or hormone therapy after surgery (if required)
  • Long-term follow-up for recurrence

Hyperthyroidism

  • Usually treated with medication or radioactive iodine
  • Surgery may be considered if medication isn’t effective or the gland is very large

Hypothyroidism

  • Treated with daily thyroid hormone replacement
  • If there are structural issues, further evaluation with scans or a biopsy may be needed, and surgery could be considered in certain cases

  • A lump or swelling in your neck
  • Persistent hoarseness or voice changes
  • Difficulty swallowing or breathing
  • Recurrent cough without other causes
